I do not file insurance claims, but I can give you a statement you can file with your insurance company for reimbursement. Payment is due at the beginning of each session.

 

Cost per session - $125

 

Sliding Scale – A Sliding Scale is available

 

Payments Accepted – Most credit cards, including HSA cards, cash, and checks. There is a $25 fee for returned checks that will be payable before a subsequent session will be scheduled.

 

Missed appointments – A $50 cancellation fee will be charged if an appointment is missed without notice. I realize last-minute reasons for missing appointments can happen and if I am notified prior to the appointment time, the fee will not be charged
